The REBUILDER PROGRAM:
What is it?
We are now offering a rebuilder program for individuals who have completed his/her physical therapy program but are still suffering from peripheral neuropathy symptoms. The programs consists of 30 minute rebuilder treatments focused on reducing Peripheral Neuropathy symptoms
How much does it cost?
The cost of participating in the program is simply $10 a 30 minute session.
What do I need to do?
1) You must have completed your physical therapy program at Ultimate Physical Therapy.
2) Have a current script to clear you for participation in the program
3) You must call to reserve an appointment
4) Have paperwork filled out and signed prior to starting program

2) Continue to treat your neuropathy symptoms with the rebuilder after your PT has concluded
